Early review means a review which is not as to scheduled plan for example if your plan is 1 year and you are reviewing after 3 or 6 months.

Do not complete the below form if you require a functional capacity assessment.

If you are reviewing early beacuse of insufficent funding, you may need a functional capacity assessment to evidence need.
