This agreement outlines the rights and responsibilities of the client and/or their representative regarding provision of allied health services with Transforming Kids Therapy Services.
Under the NDIS Practice Standards, we are required to have a service agreement in place with each participant which establishes expectations, explains the supports to be delivered, and specifies any conditions attached to the delivery of supports, including why these conditions are attached. However, Transforming Kids acknowledges how service agreements are vital with all clients in ensuring the above, and enabling greater informed choice and control, and thus Transforming Kids chooses to have a service agreement in place for all clients.
